Organizational Pre-Assessment
In our continued effort to support the nonprofits that attend the Healthy Community Institute (HCI) we ask that you take 5-10 minutes to answer a few questions about your organization. This survey is the second step in the registration process and is required in order to confirm your participation in HCI. Please complete the assessment just once for your organization.
