HCA HOME CARE AND HOSPICE AWARD NOMINATION FORM

HCA is accepting nominations for our Annual Awards as part of HCA's 44th Annual Conference on May 4-6 in Saratoga Springs. A summary of each award is provided below. Please read the summaries and then select a nominee for any of the six categories by completing the rest of the form. Nominations are due by March 14th. This is a great way to recognize your staff, your agency or an exceptional colleague. Please make your nominations today.

CARING AWARD
This award recognizes a professional and/or a paraprofessional staff person of a member agency who has exhibited the compassion, skills and service that sets their contribution apart, or whose actions on a particular day, or over a period of time, exemplify outstanding and exceptional compassion and caring for their patient(s).

ADVOCACY AWARD
This award recognizes a provider agency or person (including both elected and appointed officials) who took risks or “stuck their neck out” to improve the home care industry, clients, and workers through their advocacy efforts.

QUALITY & INNOVATION AWARD
The "Triple Aim" demands that home care and hospice agencies, payors, and other providers work to: improve patients’ experiences of care (including satisfaction); address the health of populations; and reduce per-capita costs in the health care system. These three pillars have long been implicit values of the home care and hospice models, but providers are increasingly challenged to put their clinical programs and resources to work in achieving and demonstrating these goals. This HCA award recognizes a home care or hospice provider agency, Managed Long Term Care plan or team that has implemented a concrete, innovative clinical model, workforce supports, patient or staff education program, or other initiative with demonstrated outcomes in terms of client satisfaction, population health improvement, and cost reduction or avoidance.

PHYSICIAN HOME CARE CHAMPION AWARD
Doctors play a vital role in the authorization of home care services, from the initiation of physician orders for home care to the documentation of medical necessity and engagement with home care agencies in the management of the patient’s care. Truly visionary physician practices, however, are stepping up to new levels in facilitating home care services to their patients, seizing new and innovative opportunities, and pursuing collaborations like the HCA-developed homecare-hospital-physician statute, Accountable Care Organizations, and other fulfillments of the "Triple Aim." This award recognizes a physician in New York State who has demonstrated a strong and model commitment to home care patients and agency partners. The awardee will have engaged in an exemplary fashion with home care providers on: care management at home (through effective care-transitions, primary care house-calls, home care-directed telehealth, and other means); specific community health interventions that directly involve collaboration with a home care agency; and/or other home care-supportive leadership roles.

PATIENT "SHINING LIGHT" AWARD
The entire mission of home care and hospice is centered on the patient and the quality care and support entrusted to providers. The persons we serve are the ultimate calling of our field. They never cease to amaze, teach, and inspire us. This award is given to a patient who has confronted medical challenges or circumstances in a way that has achieved far-reaching inspirational and exemplary effects. This person stands apart from others in one or more ways: displays tremendous courage in facing their medical challenge; inspires other patients, caregivers and/or health or community leaders; educates and/or advocates for patients, families and caregivers; and/or acts as an ambassador for home care or hospice in advocacy by providing testimonials, acting in an advisory role, speaking at public forums, or providing outreach to officials and leaders. Special consideration will be given to nominees who triumphed over COVID-19 illness as well as patients who, through the assistance of home care, overcame medical or social circumstances uniquely challenged by the pandemic.

RUTH F. WILSON AWARD
HCA’s highest honor, the Ruth F. Wilson Award, is bestowed upon an individual who has demonstrated exceptional dedication and made a significant contribution to home care over a period of time. With this award, HCA recognizes someone whose work has had a dramatic effect on the home care industry, reaching beyond a single agency. The recipient will have strengthened the practice of our profession, raised the standards of quality and care for patients, or advanced the level of understanding and mutual cooperation among the home care community, legislators, federal and state agencies, health care providers and the public. This prestigious award recognizes an individual who has made our services stronger and more visible and who has never wavered from his or her support of home care.
