1. Survey Purpose and Overview

This survey will provide information to Mentor Washington about your mentoring program as part of the 2021 grant opportunity funded through the Washington Health Care Authority, Division of Behavioral Health & Recovery (DBHR). The DBHR Request for Application (RFA) requires applicants to be designated as existing evidence-based programs or to consult with MENTOR Washington regarding implemented best practices for community-based mentoring.
 
DBHR restricts funding for community-based and School-based mentoring programs only.  
 
This best practices survey should only take 10 minutes or so to complete.

Next Steps:

Upon submission and if awarded, DBHR will consult with MENTOR Washington to review ALL survey responses to determine if:

  • the applicant’s identified existing program qualifies as an evidence-based mentoring program; or
  • if the applicant will need additional technical assistance from MENTOR Washington to improve its existing mentoring program for compliance with the Effective Elements of Practices for Mentoring.
