Instructions & FAQs

At the end of this application, you'll be asked to provide the following additional documents:
  • a list of your organization's programs,
  • a breakdown of your organization's funding by type,
  • your latest audited or reviewed financial statement, and
  • a financial report generated by your accounting system, separated by program / funding source.
