Parent Foundations Survey

The purpose of this survey is to find out what parents/families think. There are items about safety, how students interact with each other, how students and staff members interact with each other, the rules and expectations for student behavior, how students feel about the school in general, how staff members interact with parents/families and potential problem areas that might need to be addressed.

Please respond to every item. Base your responses on the experience of both you and your child(ren).

Important! This survey is anonymous. If you do have specific questions or concerns please include them at the end of the survey in the Additional Comments section with your name, if desired.
