USD 481 Needs Assessment for 21st Century Grant

USD 481 is applying for a 21st Century Learning Grant through the Kansas Department of Education. The grant could provide an opportunity for the district to offer after school and summer programs that both include enrichment and academic activities at no cost to our families.

In order for the district to apply for this competitive grant, it is required that a needs assessment of the families and students attending the school(s) be completed. This assessment survey allows us to identify which program type (after school and/or summer) would be of most benefit to our district's families and helps us to learn about your needs.

The survey will take less than 10 minutes of your time. As you complete the survey, please think carefully about your circumstances and how such a program would best serve you and your child(ren).
