* 1. As we have run out of space in our current building, we are intending to move to Level 2, Chester Street Car Park, Douglas, IM1 2PG. This is the site of the old Loch Promenade Surgery which can be accessed from level 2 of the car park (free 2 hour disc parking for visiting patients) or via the lift on Chester Street, opposite Strand Shopping Centre. Please indicate your level of satisfaction with this proposal.

* 2. The practice will continue to be known as Finch Hill Health Centre and will be run by the same staff, Doctors and Nurses. You will continue to be registered with the surgery and will not need to do anything apart from be aware of when the move is due to take place. Patients wishing to re-register with another practice will be at liberty to do so, according to existing practice's catchment areas.  Please indicate your level of satisfaction and any comments regarding this proposal.

* 3. Finch Hill Health Centre's catchment area will remain the same. We will continue to provide the same services as at present, including minor surgery, joint injections, family planning and phlebotomy, immunisations, antenatal and child health checks. Core opening times and access to booking appointments, including by telephone and on-line access will not change. The surgery will plan to move over a weekend to ensure minimal disruption to services. Please indicate your level of satisfaction with this and any comments.

* 4. Our patients' paper records will be moved to the new site where they will be stored in a secure medical records room, only accessible by Finch Hill Health Centre staff. There will be no change to the current ordering and collection of prescriptions. Please indicate your level of satisfaction with the current arrangements and any comments.

We will provide you with regular updates regarding the proposed move on our practice website and there will be a formal announcement from the Department of Health and Social Care when a date has been agreed. It is anticipated that this will be mid-2021.
