Question Title

* 1. A 68 year old man with history of CABG 6 years ago presents with worsening fatigue. He is treated with furosemide 80 mg bid, lisinopril and metoprolol. Exam shows jugular distention to mid-neck in the upright position, 2+ peripheral edema, clear lung fields, and a soft holosystolic murmur at the LLSB. Hemoglobin is 10.6, creatinine 1.9, BUN 56, NTproBNP 456. Echo shows normal LV size, LVEF 60%, PASP 40, IVC is dilated and not collapsing, lateral e/e’ ratio is 12 and septal e/e’ is 8. Chest film shows no pulmonary congestion and small bilateral effusions. ECG shows sinus rhythm at 72 bpm with LAE. What do you recommend?
