1. This survey is for people who work or volunteer for a CJG.

We respectfully acknowledge Aboriginal and Torres Strait Islander peoples as the First Peoples of Queensland and their continuing connection to land, sea and community. We pay our respect to their Elders, past, present and emerging.

This is the third and final year of the CJG Program Evaluation Survey (2021 to 2023). The survey asks about the outcomes and impacts of CJG services for Aboriginal and Torres Strait Islander people and communities.
 
This survey is intended for:
- CJG coordinators
- other CJG staff members
- Elders and Members of the CJG.

You can respond on behalf of your CJG or as an individual. If you have been involved with more than one CJG, please give an overall assessment in your rating. You can make specific comments in the text boxes.

Your feedback is very important for the evaluation. No personal information is collected and your response is completely private and confidential. You can skip questions or stop at any time.
