This survey will include all CUSD Child Development programs (Carmelo, Carmel Adult School Co-op, Apple Pie, and Cachagua Children's Center).  If you have more than one student enrolled at any of the Carmel Child Development Centers, please complete a separate survey for each child. If you have a student enrolled in TK or K-12 please complete the District parent survey that was sent via email in addition to this survey.
Carmel Child Development Programs conduct this Family Survey as part of its self-assessment process.   Family perspectives are essential to this process. All individual answers will be anonymous.
