Welcome to My Survey

Thank you for participating in this survey. We appreciate your time and patience. You are being invited to take part in a research project by Ms. Monica L. Johnson, Mrs. Brittany Falcon Rutland, and Dr. John Tetnowski from the University of Louisiana at Lafayette. The purpose of this study is to explore people who stutters’ experiences with treatment and the factors that influence attrition. You were selected to participate in this study because you are or have ever been identified as a person who stutters and have or are currently attending speech therapy. You will be expected to complete a web based survey and should you consent, participate in a follow-up interview.

The survey consists of a series of general and specific questions. You may choose not to answer any question that makes you uncomfortable, or that you want to skip for any reason. The entire survey will take about 15 minutes and will need to be completed in one sitting.

At the end of the survey you will be offered an opportunity to provide your feedback on the study. If you would like to be contacted regarding a follow-up interview further exploring the concepts of the survey please provide your preferred method of contact. The follow-up interview will last approximately one hour and will take place at the University of Louisiana at Lafayette Speech and Hearing Clinic or through a HIPPA compliant video conferencing platform with a doctoral researcher. 

There is no more than a minimal risk from being in this study and you will not benefit personally. However, we hope that others may benefit in the future from what we learn as a result of this study. The major risks to you are inconvenience and/or possible unintentional identification as a person who stutters due to participation in the research study.


The results of this research will be presented at conferences such as the American Speech- Language-Hearing Association national convention and may be submitted for publication in a peer-reviewed professional journal. To ensure confidentiality all data will be stored in a secured location and personal information or identifying information regarding your identity will be removed and will not be included in any of the reports.

Taking part in this research study is completely voluntary. You can decide not to be in this study, or stop participating at any time.
