Welcome to the Office for Disability Issues Stakeholder Survey 2021
Kia ora koutou,
On an annual basis at this time of the year ODI, as one of its accountabilities, needs to seek feedback from stakeholders on the work of the Office. Last year this survey was done in July due to COVID-19 delays.
The feedback is much more than an accountability mechanism, it provides the opportunity for ODI to learn from the feedback provided. We invite and welcome your feedback through this voluntary survey.
The survey should take approximately 6 minutes to complete. There are 18 short questions.
There are a few questions that you must answer. These are marked by an asterisk next to the question.
The deadline for completing the survey is 30 June 2021.
Your results will be summarised with other survey respondents, and you will not be personally identified. Please do not add any personal information that may identify you in your answers.
The Office for Disability Issues plans to share these collated results with its key stakeholders. Individual data and responses will not be shared. Information will be held within Survey Monkey, and only used for this stakeholder survey.
