10.Please add any additional information here, including how many hours you would like to request for each programme. Please note that a minimum programme length of 2 hours is recommended. We cannot guarantee staffing for programmes that are less than 2 hours in duration.

12.If you would like to cancel any LM provision that you have had during 2024-25, please enter details here. Please note, that all programmes will automatically roll into the next academic year unless they are cancelled by 23 May 2025.

13.Reasons for cancelling programmes (optional - but it is useful for us to have feedback). Please note that if you have approached a Lewisham Music tutor to work directly for your school that you should inform us of this as there is a Tutor Contract Restriction clause in the terms and conditions in the SLA that schools hold with us.
