Before you start a new My Life My Choice Exploitation Prevention group, please complete the form below to request a Group ID Number. If your group will be held remotely with youth together, but facilitators remote (i.e. at a group home, with facilitators from a partner organization on video), a staff person from the host organization needs to be in the room with the group for safety reasons.
