To apply for an event sponsorship from Group Health Foundation, please complete the following Event Sponsorship Request and Community Learning Survey. There are also two requested attachments detailed on the last page of the survey. The request form and survey are also available to complete and submit as Word documents or PDF fillable form. Alternatively, you can provide the required information in a request letter.
