Green Care/FAAT: Rural Community Survey

INFORMED CONSENT FORM

RESEARCH PROCEDURES
This research is being conducted to determine to what extent you are aware of Green Care and Farm-Assisted therapy, and how you feel it can affect those with mental disorders. It will also ask how you feel your own farm animals contribute to your health.

If you agree to participate, you will be asked to complete a survey using “Survey Monkey”, which asks only six questions regarding Green Care and Farm-Assisted therapy. The survey will take about two minutes to complete.

RISKS
There are no foreseeable risks for participating in this research for you.

BENEFITS
There are no benefits to you as a participant.

By participating in this study, you will be contributing to a body of knowledge that may influence the future use of Green Care of Farm-Assisted therapy.

CONFIDENTIALITY
The data in this study will be confidential, your email or other identifiable information will not be released or used in the reporting of the data. While it is understood that no computer transmission can be perfectly secure, reasonable efforts will be made to protect the confidentiality of your transmission.

PARTICIPATION
Your participation is voluntary, and you may withdraw from the study at any time and for any reason. If you decide not to participate or if you withdraw from the study, there is no penalty or loss of benefits to which you are otherwise entitled. There are no costs to you or any other party.

You must be 18 years or older and own farm animals to participate in this study.

If you complete the survey and provide your email address, you will be entered into a drawing with a chance to win one of two $25 Amazon gift cards.
