This is a survey about the Lafayette community response to the 2015 shooting at the Grand Theater on Johnston Street.  We hope to find better ways to respond to such tragic events.  We would like to better understand the services you provided.  We can not pay you for your participation but it would help us understand how better to respond in the future.  This survey is completely anonymous so you can withdraw at any time without any penalty.  Survey time: Less than 10 minutes.
