Dear Grad II students,
Thank your for participating in Grad II! Please read these instructions carefully.
1. DEADLINE: Please complete this evaluation by FRIDAY, SEPTEMBER 14. You may enter the survey more than once, but we need your final responses by September 14.
2. PLATFORM: Survey Monkey surveys are only optimized for use on desktops, laptops, and iPhones, iPod touches and iPads. Do not complete this questionnaire on other media or hand-held devices such as Blackberry, Treo, Palm, etc
3. CLE DOCUMENTS: Within six weeks after you submit the evaluation, Michele will email you four documents (attendance record for CLE credit, course syllabus, staff list, student list) you may need to apply for CLE credits. While we need your name for CLE purposes, your responses will be anonymous when they are shared with TLC's staff and board of directors.
4. EVALUATION SUBMISSION: To submit your responses, click the "Done" button at the bottom of the evaluation. (You are required to answer all questions, except for question #2. Error messages will be displayed in red font. Errors must be corrected before your responses can be submitted.) When your browser loads the TLC homepage, you will know your responses have been successfully submitted.
