Survey to investigate heart disease in Gordon Setters

Introduction

This survey has been created due to the increasing concerns that Gordon Setters may be suffering from early onset heart disease. Before we can understand more about this condition we need to understand more about how frequently Gordon Setters suffer with heart disease in general. The School of Veterinary Medicine and Science at the University of Nottingham is therefore conducting this survey into general heart disease in Gordon Setters. This survey will gather information on the nature of the heart disease including the clinical signs that your dogs suffered as a result of their condition along with the response to treatment and the outcome. By completing this questionnaire, you will be providing us with valuable insight that will enable us to determine how we may best undertake future research into any specific heart condition that occurs in Gordon Setters and hopefully provide better treatment options and possibly a way to eradicate the disease. This research has received ethical approval from the School of Veterinary Medicine and Science Ethics Committee, The University of Nottingham. We thank you in advance for volunteering your time to contribute towards this project by completing the following questionnaire. The survey should only take around 15 minutes to complete. Please complete one questionnaire for each of your Gordon Setters that has suffered with heart disease.

I understand that participation in this study is voluntary and that I may choose to no longer be part of the study at any time (without needing to provide reasons for doing so)

I agree that information I give during the study can be used in a report, a published paper or a conference or meeting presentation

I understand that the study is being conducted for the purposes of research

I understand that I can request to see a summary of the findings, and I can also request to see any notes made during the process of my data collection

I agree for the details of my dog included in this study to be used for other research into Pedigree dogs conducted by the researchers at Nottingham Veterinary School and their collaborators

I understand that data I have provided about my dog will be stored as part of the ongoing research into the health of Pedigree dogs. I understand this will be stored (on protected hard drives) and access is restricted only to the researchers involved in the project.

I understand that I may contact the researcher or supervisor if I require further information about the research (sv-dogs@nottingham.ac.uk) and that I may contact the Research Ethics Coordinator of the School of Education, University of Nottingham, if I wish to make a complaint relating to my involvement in the research.
