The Garrett County Health Department invites you to participate in a brief survey on medication storage and disposal. You may participate in this survey if you . . .
- are 18 years of age or older and
- live or work in Garrett County
Your comments will help us evaluate our efforts to address opioid misuse. The questionnaire will take you about 5 minutes to complete.
Your participation in the survey is completely voluntary. Your answers are anonymous and will be used by staff of the Garrett County Health Department to improve our educational programs. You may refuse to answer any of the questions and you may stop the survey at any time by closing your internet browser window.
There are no risks to you to participate in the survey other than your investment of time to complete it. There are no direct benefits to you to participate in the survey. The benefits are to the community as a whole in improving education on medication storage and disposal.
