The Montgomery County Board of Developmental Disabilities Services (MCBDDS) would like your opinion regarding the quality and type of services it has offered during the past 12 months to you or your family member. We would like individuals receiving services to complete this survey as independently as possible, with family members/guardians completing a separate survey. You can take the survey online at https://www.surveymonkey.com/r/GZPKNNS or download a copy to print at https://www.mcbdds.org/survey.

This survey has 17 brief questions and should take no more than 3 minutes to complete. Please answer all that apply to you, and return it no later than Monday, June 1, 2025.

If you wish to print and return the survey via U.S. Mail, please send it to: MCBDDS Communications Dept., 8114 North Main Street, Dayton, OH 45415. If you receive this survey from a provider, you can return it to your provider, and they will forward it to the Board.
