Purpose / Intent of Survey
Thank you for your participation in this pre-summit survey on telehealth. In preparation for the Hawaiʻi Maternal Telehealth Summit on August 9, 2023, we would like to hear from you about your patient demographics, maternal telehealth service needs, and current capacity for telehealth at your site. This pre-survey might not definitively encompass all of the topics that will be discussed at the summit, however the main purpose of this survey is to get a baseline understanding. The survey should only take 5 minutes, and your responses are completely anonymous.
