Informed Consent: Gender and Ethnic Diversity in Communication Sciences and Disorders

Dear Participant,
I am a graduate student currently enrolled in the master’s program in speech-language
pathology at Saint Xavier University. As part of the program’s curriculum, I have the option to complete a research study for a thesis. I have chosen to complete a thesis on the topic of gender and ethnic diversity in speech-language pathology.
The purpose of the study is to Investigate the factors perceived to be contributing to a lack of gender and ethnic diversity in the field of speech-language pathology. I will be conducting the research study from September 19th, 2021, to May 15th, 2022. The research study will be conducted through Survey Monkey on which you will be invited to complete an online survey. The survey should be completed before May 15th, 2022. If you are interested in participating in the study, please click on the URL link, complete the associated survey, and click on “submit”. The data gathered from the survey offers no risks of any kind to you.
All information gathered from the survey will be kept completely confidential. Information gathered from the survey will be grouped together in the report so that no individual can be identified. All survey data will be password protected. The report generated from the survey will allow me to share what I have learned because of this research study with other professionals in the field of speech-language pathology.
If you choose not to participate, the information you provided will not be included in any reports.
