Introduction: What is this survey?

We are Visible Health, a startup exploring ways to help parents get better and faster care for infants and toddlers with medical conditions.

We believe that parents are an essential resource for other parents, especially when it comes to decision-making about their child's care. Over months and years, parents develop essential knowledge about their children's health and approaches to treatment.

This knowledge is something we would like to carefully and safely gather and share. We know the exchange between parents happens on places like Facebook but we think there are more valuable and accurate approaches. We appreciate any information you choose to share and do not require answers to most of these questions. This survey has nine pages should take you 15 - 20 minutes to complete.

This may have causes that are genetic, congenital (observable at or before birth), acquired (due to events after birth), or unknown.
