Intake Form: The Black Neuroinclusive Youth Workforce Transition Model Development

Thank you for taking the time to complete this intake form. Your input is highly valued and will meaningfully inform the design and delivery of programs that support Black neurodiverse youth during their transition into the workforce.
This research project centers the voices and lived experiences of Black neurodiverse youth (ages 16–30) and the people who support them including families, community organizations, employment service providers, and employers across Canada.
We are committed to protecting your privacy and the confidentiality of your information. All data collected through this form will remain strictly confidential. We will not share your responses publicly, and we will not link any personally identifying information (such as your name or initials) to any interview or data shared.
Honorarium Details:

Black Youth Participants: $100.00 gift card
Parents/Caregivers: $50.00 gift card
Community Organizations: $25.00 gift card
Employers/Employment Service Providers: $25.00 gift card
