1. Consent to participate in the Study

7% of survey complete.
You are being invited to take part in an original research study investigating the emotional impact of having experienced an ectopic pregnancy. We are also interested in the type of support that you would have found helpful from the health service or independent sector. We hope that in the future, the knowledge gained from this study will help support other women and shape service delivery.

You are eligible to take part in this study if you are a woman who has experienced an ectopic pregnancy within the past two years. Please only complete this questionnaire once. You are very welcome to contact the ectopic pregnancy trust or the research team (maria.jalmbrant@slam.nhs.uk) if you have any questions about the study or want to discuss anything further before you decide to take part.

The study will take approximately 15-20 minutes to complete. Some women may find some of the questions upsetting: please discontinue the questionnaire if this is the case.

This is an electronic form of consent for the study. By ticking the boxes below, you agree to take part in the study.

You will not be asked to provide any personally identifiable information in the questionnaire and your responses will be kept confidential.

Participation in this research is voluntary and you may withdraw at anytime by not answering any further questions.
