SFDPH Workplace Safety & Violence Prevention Survey

This survey is designed to support the San Francisco Department of Public Health’s obligations under Cal/OSHA, SFDPH HR-1, and the SFDPH Workplace Violence Prevention Plan (WVPP) by identifying workplace safety risks, workplace violence exposures, injuries, environmental hazards, and organizational response gaps. Survey findings will be used to inform prevention strategies, training, corrective actions, and system-level improvements to protect employee health and safety.
