City of Berkeley Recreation Division Cancelled Program Transaction Request Survey

To efficiently process requests for refund, credit on account, or donations, we have developed the following brief survey to route your request. Please complete the questions below. If you have any questions or would like additional information, please contact our Recreation Administration Office at (510) 981-5150 or recreation@cityofberkeley.info. To review your currently enrolled programs, activities, or registraitons, you can log in to your City of Berkeley Recreation Division Account here:

Please note City Staff have already begun processing full refunds for most programs and registrations scheduled between March 16 and April 7, 2020. This survey pertains to programs taking place between April 7 and May 3, 2020.
