By submitting this form, your information will go to the Nominating Committee who will review submissions and may reach out for additional information. This is not a guarantee of being on ballot; per our Bylaws, the nominating committee creates the final ballot based on submissions of members. Form is open from Tuesday, Jan 4 - Friday, Feb 4th. Notification of acceptance by Feb 7th.
