New muscle aches (myalgia) or extreme fatigue that cannot be attributed to another health condition or specific activity (such as physical exercise)?
A new loss of taste or smell or an unexplained change in cognitive function?
Received a positive test result for the virus that causes COVID-19 within the past 10 days?
Been in close contact (within about 6 feet for 15 minutes or more) with someone with suspected or confirmed COVID-19 in the past 14 days?
