Atikokan General Hospital (AGH) needs your help in shaping our future as your rural hospital/healthcare provider, employer, and partner. AGH wants to hear from you!

Why are we doing this survey?

·        We want to make sure that our priorities are guided by the voice of our patients / residents / clients, employees, partners, and community members

What are we going to do with the information? We will…

·        collect all information and put it all together, pull out common themes from the survey

·        schedule group discussions to make sure we understand your message clearly

·        share the result of the survey and use the information to guide our strategic plan

·        Share any identifying information or examples that can identify a particular person

·        AGH Leadership and staff                                                     First Nations Partners

·        AGH Patients / Residents / Clients / Family Members      Physician Partners

How do you send your completed survey back to AGH?

·        Completed surveys can be dropped off at the main reception area, Extended Care Wing, Acute Care, Rehabilitation Department, and Community Counselling Centre

·        Partner organizations can also receive copies of your completed survey
