The Department of Elementary and Secondary Education (ESE) is in the process of developing an integrated suite of tools for educators. One of the tools is an Early Warning Indicator System (EWIS) that will be for grades Kindergarten through 12 (with the possibility of expansion to early education programs). The EWIS will provide student-level and aggregate information on students' risk level for falling off-track - defined as not meeting an expected academic milestone.

Over the coming months there will be opportunities for educators to learn more about the risk models being developed and provide feedback on the EWIS (model, user interface, trainings, etc.). Please complete the short survey below if you are interested in being on our EWIS contact list! You will be notified of all upcoming EWIS-related opportunities if you complete this survey, however you will not be obligated to participate. We look forward to working with you on this important tool.

* 1. Please provide your name and contact information:

* 2. The new EWIS will cover a large span of ages. What educational levels (age groupings) are you most concerned about? Please check all that apply.

* 3. What components of the EWIS project are you interested in learning more about and/or providing feedback on? Please check all that apply.

* 4. Please let us know if you have any comments or questions that you would like us to address in the forthcoming information about the EWIS.
