REQUIREMENTS FOR ANNUAL DISCLOSURE FOR CME ACTIVITIES

As a physician or non-physician participant in a planning committee, case discussant, or faculty presenting CME activities in the South San Francisco Service Area, the South San Francisco CME Program is required to obtain your disclosure of any financial relationships with a commercial interest (CI) within a 12 month period preceding the CME activity.

A "commercial interest" is defined by the ACCME/CMA as any entity producing, marketing, re-selling, or distributing health care goods or services consumed by, or used on, patients. The ACCME/CMA does not consider providers of clinical service directly to patients to be commercial interests. For more information, visit www.accme.org.

The ACCME/CMA considers financial relationships to create "conflicts of interest" when individuals have both a financial relationship with a commercial interest and the opportunity to affect the content of CME about the products or services of that commercial interest.

Question Title

* 1. Your (including your spouse/partner) disclosure information will remain in effect for all CME activities from January 1, 2020 through November 30, 2020; unless your relationships changes, at which time an updated disclosure will be required.
