As part of the Georgia O'Keeffe Museum's (GOKM's) 2-year grant-funded project, "A New Way of Seeing: Building Capacity Through the Lens of Inclusivity," this form provides an opportunity to share your feedback at any point during the project. This form has been created for use by GOKM staff, board members, project consultants, and any other key stakeholders on the project.

It is suggested you use this form to:
- reflect on how the project is going so far,
- share successes and/or challenges along the way,
- offer compliments and/or concerns, or
- offer feedback and suggestions at any time.

You may choose not to use this form at all, or you may choose to use it often. Each time you use this form, you will be able to choose whether you want to stay anonymous (not sharing your name or any identifying information) or if you want to attach your name to it. It is always entirely voluntary and up to you. 

While there will be several other surveys and evaluation mechanisms throughout the project, most will be tied to specific project activities (e.g., evaluating the public programs, staff workshops, etc.). This online form is a place to share things that might not be captured by those more targeted evaluation mechanisms. It also allows you to share exactly what you want to share, when you want to share it. 

Responses to this form will be accessed and read by the project's external evaluator, Kate Livingston (of ExposeYourMuseum LLC). You will have several choices below related to how you want your feedback to be shared beyond the external evaluator, if at all.
