My diagnosis - what it means for me.

Introduction

Welcome to our Lifestyle Health Foundation "My Diagnosis" questionnaire, being run in partnership with the Person-Centred Neurosciences Society, who are running the related "My Neuro Diagnosis" survey.

The purpose of both surveys is to collect anonymous thoughts and data, that will encourage, support and shape the design and provision of more person-centred emotional health services.

This "My Diagnosis" survey is focused on questions relating to a diagnosis of non-neuro longterm conditions e.g. Type 2 diabetes, asthma, Crohn's disease, hay fever, PCOS.

If you have been diagnosed with more than one longterm condition, please begin the survey answering in relation to the diagnosis of that condition you consider to be the most impactful for you. You have the opportunity to answer the same questions, for a second and any further diagnoses, if you wish to.

Should one of those diagnoses not be covered by this survey, such as a neuro or mental health condition, you are invited to complete the P-CNS questionnaire (if you haven't already done so) at https://www.surveymonkey.co.uk/r/FRLSQDW
