Thank you for attending the LGBTQ+ Health 301 webinar series! This is the evaluation for Delving into Consensual Non-Monogamy: Including Family and Relationships Structure Diversity in Health Care.
Please take your time to complete this webinar evaluation. Evaluations allow GLMA to assess how we met your continuing education needs as well as to develop better programming in the future.
Evaluation Requirements:
Although email and name are optional, you must provide both in order to receive a CME/CE certificate (you are able to provide this information later in the survey).
Names and emails will not be associated with evaluation responses.
Please complete this evaluation no later than April 28, 2023. CE certificates will be emailed to you in early May.
To be eligible to receive CE credits, you must complete this evaluation and indicate your name, your GLMA membership status, and contact information at the end of the survey.
Continuing Education Certificates:
GLMA is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education (CME) for physicians.
For non-physicians, GLMA is accredited through the American Association of Sexuality Educators, Counselors and Therapists (AASECT) and is approved for CE credits. These CE credits may be applied toward the renewal of AASECT certification and may be accepted by individual state licensing boards for licensure renewal. Individuals are encouraged to reach out to their state licensing board to check whether AASECT CE count towards CE hours for licensure renewal.
GLMA designates this webinar activity for a maximum of 1.5 AMA PRA Category 1 Credits (TM) per webinar. Completion of this evaluation will be matched against attendance records of the webinar.
Cost: If you are an active GLMA member, the CE certificate will be no cost. If you are not an active GLMA member, you will be contacted to make a $25 payment to receive your certificate.
