2019 GLMA Annual Conference Evaluation

Thank you for attending the 37th GLMA Annual Conference on LGBTQ Health in New Orleans.

Please take your time to complete this conference evaluation. Evaluations allow GLMA to assess how we met your continuing education needs as well as to develop better programming in the future.

This is also an opportunity for GLMA to collect demographic information about conference attendees to ensure conference content accurately reflects the professional needs of all attendees.

Although email and name are optional, you must provide both in order to receive a CME/CE certificate (you are able to provide this information later in the survey). 

Names and emails will not be associated with conference evaluation responses.

Continuing Education Certificates
To be eligible to receive CE credits, you must complete evaluations for each session you attended and indicate your name, number of credits you are claiming and contact information at the end of the survey. There is a $35 fee for issuance of CE certificates. If you have not already paid the certificate fee, you may do so at this link or go to http://bit.ly/GLMA19_CE on your computer.

Please complete this evaluation no later than November 30, 2019. CE certificates will be emailed to you.
