The Nebraska Heritage Health Stakeholder Coalition, made up of service and medical providers across the state, continues to work with Heritage Health to find resolution and create an easier means of assisting providers in providing services through the three Nebraska Medicaid Managed Care Organizations. 

We would appreciate you taking the time to complete the survey below to provide information on the levels and issues that providers across the state are working on in reference to the Heritage Health Managed Care Plans. Please note that all responses will remain anonymous. We would encourage all providers participating in Heritage Health to provide a response in an attempt to have data that is representative of a majority of providers across the state.
