Background information

Why complete this  survey?
 
This is the THIRD check-in with members about Quality Improvement structures and processes in teams.  The data from these surveys is making a huge difference.  The results from this survey will be used to show the continuing progress AFHTO members are making in primary care performance.  With these data. AFHTO members will be better able to answer questions like: 
1) What do teams that have really great patient outcomes do differently in terms of team dynamics, EMR use and/or board structure?
2)  What is having the greatest impact on team engagement and participation in QI?   
3) How much better are we doing with using EMR data to improve, even if EMRs might not be changing that much yet?

 Who is being asked to complete this survey?
EDs from all AFHTO member teams are asked to respond.  It doesn’t matter whether your team contributed data to D2D or not. This request is directed to EDs only to make sure each team has only one response. EDs are more than welcome to consult with or delegate this to their teams in completing the survey. 

What is the connection to D2D? 
The survey helps us understand the usefulness and impact of D2D.  That's why it is helpful for EVERY team to respond, even if you didn't contribute data to D2D.  The survey also helps us understand what it takes to get to the highest levels of performance.   That's why it is really helpful to include your D2D team code if your team contributed data to D2D. 
 
What does anonymous REALLY mean for this survey?
You will NOT be asked who you are or which team you are with.  As noted above, you WILL be asked to provide the code you use for your D2D data, if you contributed data to D2D.  You can choose NOT to do this and still complete the survey. 

Even if you do provide your D2D code, this will not connect your survey results back to you.  It will allow your survey results to be linked to your D2D data so we can answer questions like those above.  It is true that if you have shared your D2D code with AFHTO, it is technically possible to trace back to you.  However, as indicated in D2D submission process, this will happen only IF and WHEN you ask AFHTO to do that (ie if you forgot your code) or if you agree to be contacted to share your story as a high performer. 

Who will get the results?
AFHTO members will get a summary of the survey results, which will be combined with D2D to answer important questions like those above.  The results will also be shared with external stakeholders (at the membership level) to demonstrate and advocate for the value that AFHTO members deliver.  All of this is also part of the doctoral research of Carol Mulder who is exploring what it takes to successfully implement performance measurement in primary care, from the perspective of those who are actually providing that care, ie primary care teams.
