Please complete this survey for each course that you currently have provisionally or fully accredited. ESSA must be informed of any planned changes to an accredited course. However, a Course Change Notification Form must be used this purpose and submitted directly to ESSA’s Course Accreditation Manager (Rachel Holmes) via email to rachel.holmes@essa.org.au.  Additional costs may be incurred if significant changes need to be reviewed.
