Thank you for your interest in becoming a Gateway to Industry School for Health!

CheckUP currently partner with a support over 100 schools across Queensland as part of the Health Gateway Project.

Please note that at this stage, schools wishing to join the Health Gateway Project will join a waiting list. If you would like to join the waiting list, please complete the expression of interest form below and a team member will be contact with you as soon as possible.

This form provides CheckUP with some initial information and assist us in identifying opportunities for your school’s engagement in the program.

In accordance with CheckUP Privacy Policy, we value your privacy and only collect necessary information to provide and improve our services while ensuring your data remains secure.
