Participant Consent
Welcome to the online survey! We are inviting you to take part in a research study because we are trying to learn more about girls’ participation in Science, Technology, Engineering and Math (STEM) programs and activities and we care what you think!
Your parents have been informed and have granted given permission for you to participate in this study concerning questions about yourself, your family, and your past and/or current participation in STEM programs and activities. This survey should not take more than 20-30 minutes to complete.
All the information that you provide in the survey will be anonymous. Your participation in this project is voluntary and you may stop your participation in this study at any time. If you choose not to participate, there will not be any consequences with respect to your current/future participation in STEM programs in any way.
