2025 Georgia Pediatrician Needs Assessment in Supporting Youth to Adult Models of Care

Annual Assessment of Pediatric Practices Supporting Youth in Transitioning to Adult Models of Care

Dear Colleague,

Developmentally appropriate care through the life of a child is essential especially when supporting youth to transition from pediatric to adult oriented care. Over the years the Chapter has endeavored to support our members in this important effort.

We request that you complete this brief survey to help us identify how best to provide you with available resources on healthcare transitions; the survey should take less than 5 minutes to complete. Thank you for your time and consideration.

Sincerely,

Nicola Chin, MD, FAAP
President
Georgia Chapter of the American Academy of Pediatrics
