LeadingAge Ohio is surveying Ohio’s adult day services providers to support advocacy work on behalf of the sector. Please answer to the best of your ability. Responses are confidential and will be reported in the aggregate only, so will not be identifiable to individual providers. All participating providers will receive the data ahead of any publication.
