Hello and welcome to Culinary Kids Academy, the most innovative and fun cooking program your child(ren) will ever experience! We use hands on cooking classes as a vehicle to teach Math, Science, History, Safety, and Nutrition! Not only do CKA students learn the valuable skill of cooking, they do so while reinforcing what they are doing in their school day. Learning to cook increases student's level of independence, responsibility, self-esteem and ability to own their own diets! THEY'RE LEARNING WHILE HAVING A BLAST! IT'S A WIN-WIN PROPOSITION! We can't wait to be with your children and thank you for letting us teach them. Expectations: CKA students are required to act in a reasonable manner when engaging in workshops. Students will be held to the same standard as if they were engaged with their school classroom teacher. These standards include being courteous and respectful of CKA and school property. Students are expected to take part in clean up procedures at the end of class. CKA reserves the right to dismiss students whose behavior is disruptive to the class proceedings or potentially harmful to other students. No refund will be provided to those violating the aforementioned policies.
Terms of Release: By signing this form, covering any and all classes (both in-person & digitally/on-line) given by Culinary Kids Academy/Culinary Kidz LLC, (the "Company"), a California limited liability corporation, P.O. Box 260891 Encino, California 91436, I hereby indicate my understanding and agreement to the following; That my contact information may be utilized so I can be informed of future fun workshop opportunities (CKA will never share email information with third parties). I further understand that there are natural and inherent risks associated with participating in any cooking activities, including, but not limited to, the risk of physical injury or harm from knives, stoves, ovens and other kitchen equipment, hot water/oil, food borne illness, and any/all allergic reactions. I acknowledge and assume any and all risks associated with the culinary activities related to the company in which my child(ren) will be participating I further agree to waive any and all claims, actions, causes of action, demands, damages, or liabilities against the Company, its subsidiaries, and related companies and their members, owners, shareholders, directors, officers, managers, employees, and agents arising from or related to my child(ren)’s participation in culinary classes. This waiver specifically includes any claims of negligence or otherwise against the Company, and/or its members, owners, shareholders, directors, officers, managers, employees, and agents, with respect to harm alleged by my child(ren) arising from or related to participation in the Company’s culinary classes. I agree that the indemnified parties will have no liability in connection with any and all loss, damages, judgments, awards, costs, expenses, and attorneys’ fees of the indemnified parties in connection with any harm or damages related to participation in the company’s culinary classes. This waiver shall remain in force and effect and apply to all classes taken by my child(ren) until revoked in writing by me and delivered to Culinary Kids Academy/Culinary Kidz LLC. As a rule, Culinary Kids Academy/Culinary Kidz LLC does not use nuts or shellfish in its recipes. Culinary Kids Academy/Culinary Kidz LLC does, however, purchase ingredients that may contain the following manufacturer’s notice/disclaimer (or the like): “Product may be manufactured on equipment also used for nuts/shellfish/other allergens.” Thus, while Culinary Kids Academy/Culinary Kidz LLC aims to avoid cross-contamination, it cannot guarantee the total absence of cross-contamination.
