The Sooke Region Communities Health Network (SRCHN) is creating an updated inventory of Mental Health, Substance Use and Addiction Services available in the Sooke Region.

Your input is greatly appreciated and this survey should only take about 10 minutes.

This survey will remain open until August 31st, 2022 and you are invited to share it with other service providers.

Please note: When we say 'Sooke Region', we mean the communities of East Sooke, Sooke, Scia'new First Nation, T'Sou-ke First Nation, Otter Point, Shirley, Jordan River, Port Renfrew and Pacheedaht First Nation.
