Safety Incident Reporting and Engagement System

KVC Missouri requires all staff to observe high standards of business and personal ethics in the conduct of their duties and responsibilities. We must also work to ensure that our environment is safe for staff, clients, and visitors. As employees and representatives of KVC Missouri, we must practice honesty and integrity in fulfilling our responsibilities and comply with all applicable laws and regulations.

SIRENS is intended to encourage and enable employees and others to raise serious concerns internally so that KVC Missouri can address and correct safety hazards, inappropriate conduct and actions.
If this involves suspected or observed incidents of child abuse and neglect, the Child Abuse and Neglect hotline should be called immediately by calling 1-800-392-3738.

It is contrary to the values of KVC Missouri for anyone to retaliate against any board member, leader, employee or volunteer who in good faith reports an ethics violation, a safety violation, or a suspected violation of law, such as a complaint of discrimination, or suspected fraud, or suspected violation of any regulation governing the operations of KVC Missouri. An employee who retaliates against someone who has reported a violation in good faith is subject to discipline up to and including termination of employment. To help further this goal reports can be submitted anonymously. Please be sure to give as much information as possible. If we cannot determine the location or nature of the concern and you have reported anonymously the issue cannot be properly investigated and will be closed.

What is reportable?

· Safety concerns

· Slips/trips/falls
· Maintenance issues that are causing a safety concern and have failed to be addressed
· Incident that didn’t result in harm, but could have
· Incident that only resulted in minor harm (scrape, bruise, broken/damaged object)
· Inappropriate staff behaviors
· Ethical concerns
· Poor lighting, locks not working or other security concerns
· Every-day annoyances that are a safety issue or could result in physical harm
· Accounting and Auditing concerns

If you are not sure if your report should go through SIRENS you can still submit and it will be evaluated. You can also discuss it with the Compliance and Safety Coordinator or utilize the SIRENS hotline by calling extension x7199.


If you would like to see what has been reported and what actions are being taken please visit the SIRENS log on the intranet. Please allow up to 10 business days for changes to be posted.
