Guelph Community Health Centre Virtual Programming Survey 

Hello there!

We would like to hear thoughts and opinions on the programs that are offered at the Guelph Community Health Centre and your technology comfort level and availability while we consider virtual program planning. This survey will help us understand if our programs would be useful and easy to access.

This survey will take about 5 minutes to complete. Your participation is voluntary. You do not have to participate in this survey if you don’t want to, and you can quit this survey at anytime. Your responses are anonymous and confidential. Your answers will not affect your ability to access the programs/services at the Guelph Community Health Centre.
