Dear Participant:

My name is Evelyn Lockhart, and I am a pathologist as well as a graduate student in the Biomedical Communication program at the University of Toronto Mississauga. For my master’s research project, I am developing an animation on primary hemostasis to be used in undergraduate medical education. My goal is to create an animation which could be used freely and widely in medical school hematology courses. To that end, I wish to hear from educators those learning objectives they believe are most important for medical students. Additionally, I would like to learn from educators what aspects of primary hemostasis may pose learning challenges for medical students.

Because you are a member of the Foundation for Women and Girls with Blood Disorders Learning Action Network (FWGBD LAN), I invite you to participate in this research study by completing the following survey. Participation is strictly voluntary, and you may refuse to participate at any time. In addition, you may skip questions to which you prefer not to respond. The survey should take approximately 5-10 minutes to complete. There is no compensation for responding, nor is there any known risk. In order to ensure that all information remains confidential, please do not insert your name or other identifiers (such as your institution’s name or exact geographic location).

Thank you for your time in completing this survey. The data collected will provide valuable information regarding hemostasis education in medical school. If you have any questions regarding this survey or the animation project, please feel free to contact me.

Sincerely,

Evelyn Lockhart, MD
