QAIHC Hall of Fame 2018

Welcome and thank you for your online nomination for the Queensland Aboriginal and Islander Health Council (QAIHC) Hall of Fame 2018. The QAIHC Hall of Fame serves as an archive of the careers and accomplishments related to the Aboriginal and Torres Strait Islander Community Controlled Health Sector for those chosen for induction.

Entry Guidelines

Nominations close 4.30pm on Thursday, 6 September 2018.

1. All nominations from Health Services must be endorsed by the Chairperson of the organisation.
2. Nominating Health Services will be requested to contribute to travel costs associated for successful nominees and their families to attend the presentation dinner.

3. The judging panel will comprise a representative of the QAIHC Board of Directors, QAIHC Secretariat and a previous Hall of Fame inductee.


Nomination Terms and Conditions
1. Nominations are open to representatives of QAIHC, QAIHC member organisations and affiliates.
2. The information contained in submissions must be accurate and correct.
3. You agree to abide by decisions made by the judging panel and all decisions are final.
4. By submitting this nomination, you do so on the understanding that you must first gain approval from:
    a. The nominee to submit their name, achievements and personal details to QAIHC where they will be collected, stored and retrieved for the purpose of administering and publicising the awards in a variety of media (digital and hardcopy) in 2018 and in subsequent years.
    b. From the Chairperson of your organisation.
5. If you choose to attend the QAIHC 2018 Awards for Excellence and Hall of Fame event in Brisbane and are required to travel to Brisbane to attend, you agree to meet all travel and accommodation costs that you require.

4. Strengthening capacity of individual family and communities through effective prevention and health promotion, and
